Once coverage is verified, we will schedule an appointment for you. Deductibles and co-pays may apply and will be discussed with you prior to your appointment.
When possible, I am happy to submit claims to your insurance provider on your behalf. Please note, however, that payment from insurance providers is not guaranteed and you remain financially responsible for all services rendered.
If your insurance carrier denies coverage, delays payment, or does not reimburse for any reason, the balance becomes your responsibility and must be paid in full.
Any remaining balance is due within 30 days of notification. If financial accommodations are needed, flexible payment arrangements may be available—please contact me to discuss options.
By scheduling an appointment, you acknowledge and accept responsibility for all charges not covered by your insurance plan.
Appointment times are reserved exclusively for you. If you need to cancel or change your appointment, at least 24 hours’ notice is required so the time may be offered to another client.
Medical Massage cancellations made with less than 24 hours’ notice will incur a $100 fee. Missed appointments without notice (no-shows) will be charged the same cancellation fee.
Exceptions may be considered for circumstances such as illness, emergencies, or severe weather conditions.
